After many years of practicing dentistry and dealing with insurance companies, Dr. Kelly has seen the need to create a discount plan to meet the needs of his patients.


Research shows that people without dental coverage who would normally go to the dentist will avoid going because they are worried about the spiraling cost of dental care and the possible financial burden.


Most small businesses and self employed people are not able to provide dental coverage because individual insurance plans are too expensive, limited in coverage, and confusing to use with many forms to complete and unexpected restrictions. Additionally, most people agree that decisions regarding their dental care are best made between themselves and the dentist – without third party involvement.


Dr. Kelly created the Dental Membership Plan to provide you and your family with affordable and efficient dental coverage that you and the dentist own and administer. No insurance company will interfere with any decisions made about your dental health. No claim forms. Only you and your dentist make the decisions regarding your dental treatment!


This plan provides outstanding coverage that will out value dental plans from big insurance companies and large employers benefit plans.

Dr. Kelly and his excellent staff will provide the best quality dental care available. We offer not only preventative dentistry but excellent coverage on major dental procedures such as crowns, bridges, veneers, orthodontics, root canals, dentures and implants. If any services cannot be done at our office a specialist will be available to provide services at a discount.


Membership Fees:

Single                                                  $59/month
Two Persons                                       $79/month
Family (up to five members)                 $99/month
Surcharge per add'l family member      $10/month

Premium payments are payable on the first of each month.  Automatic bank withdrawal authorization is required.

Member benefits are limited to $3000.00 annual maximum for each covered family member per plan year performed in our office.  In addition to the $3000.00 annual maximum, we offer $1500.00 for Orthodontic services.

Coverage percentages range from 25%-100% off of dental services.

            Preventative: 100%
            Basic/Major:   25%-50%
 
        Terms & Conditions:

 

  • All discounts are available solely through Dr. Neil Kelly, DDS, LLC.
  • Member co-payments are due at the time of service unless payment arrangements have been made prior to treatment.
  • Missed or broken appointments without 32 hours notice will be charged $25 for every 15 minutes of lost chair time.
  • Minimum membership period is 12 months and payments are non-refundable.
  • Unless approved by the dentist, membership periods will renew automatically on the anniversary date until cancelled in writing.
  • 30 days written notice of cancellation is required.
  • Family members are defined as a husband, wife and non married children living in the household.
  • Membership fees and plan discounts are subject to change on an annual basis.
  • Dr. Neil Kelly, DDS, LLC reserves all rights to cancel or discontinue this plan for any reason at the end of any month with or without notice.
